Princípio: A identificação precoce do risco nutricional permite intervenção alimentar eficaz e de baixo custo, reduzindo complicações, tempo de internamento, reinternamentos e mortalidade. Mais de 30% dos doentes internados estão em risco nutricional.
Cuidados de saúde primários (CSP)
Rastrear todo o adulto com probabilidade aumentada de deterioração nutricional:
- Idade > 65 anos
- Doença crónica que comprometa o estado nutricional (DPOC, oncológica, renal, hepática, gastrointestinal, inflamatória intestinal) não seguida em Nutrição hospitalar
- Doença neurodegenerativa avançada
- Cuidados de reabilitação, domiciliários ou doente institucionalizado
- Indicadores clínicos: perda de peso não intencional, anorexia, alterações do paladar/olfacto, disfagia, problemas de dentição/mastigação, cansaço/fraqueza muscular, úlceras de pressão, infeções recorrentes, isolamento social com ingestão comprometida
Periodicidade CSP: primeiro contacto; depois anual (sem risco, não institucionalizado) ou trimestral (institucionalizado/cuidados domiciliários).
Cuidados hospitalares (CH)
- Todos os internados por período > 24 h e doentes oncológicos em ambulatório
- Primeira avaliação nas primeiras 48 h após admissão; repetir semanalmente se não seguido pela Nutrição
- Doentes de cuidados intensivos são considerados sempre em risco (excluídos do rastreio sistemático)

Desnutrição: estado por insuficiente ingestão/absorção de nutrientes que altera a composição corporal (perda de massa magra) com declínio funcional e pior prognóstico. O rastreio (ex.: NRS 2002) precede sempre o diagnóstico.
Abordagem em 3 passos: (1) rastreio com ferramenta validada; (2) diagnóstico exige ≥1 critério fenotípico + ≥1 critério etiológico; (3) graduação de gravidade pelos critérios fenotípicos.

ATENÇÃO: Não usar a hipoalbuminémia como indicador de desnutrição (baixa sensibilidade e especificidade). A força muscular (ex.: dinamometria) não substitui a avaliação de massa muscular no GLIM. Avaliar sempre os 5 critérios para excluir o diagnóstico.
Fragilidade: síndrome de declínio progressivo em múltiplos sistemas fisiológicos, com vulnerabilidade aumentada a eventos de stress. É prevenível e potencialmente reversível — recomenda-se rastreio a todos os ≥65 anos.

Sarcopenia: doença muscular (falência muscular) definida por baixa força muscular (parâmetro-chave), confirmada por baixa quantidade/qualidade muscular; mau desempenho físico indica gravidade. Rastreio anual nos ≥65 anos ou após evento clínico adverso major.

Prevenção/tratamento (sobreposto à fragilidade): ingestão proteica otimizada, suplementação de vitamina D e exercício físico (treino de resistência).
Caquexia: síndrome multifatorial com perda contínua de massa muscular (± massa gorda) não totalmente reversível com suporte nutricional convencional, associada a inflamação crónica. Rastrear perda de peso e anorexia em todos os doentes oncológicos.

NOTA: Sarcopenia, desnutrição e caquexia partilham a perda de massa muscular mas diferem na etiologia. A perda de peso não é critério de sarcopenia (é de desnutrição e caquexia). A desnutrição é forte preditora de sarcopenia. Podem coexistir, sobretudo no idoso oncológico.

- Diagnóstico de desnutrição (GLIM): ≥1 critério fenotípico (perda de peso, IMC baixo, massa muscular reduzida) + ≥1 etiológico (ingestão reduzida, inflamação). Não usar albumina.
- FRAIL Scale ≥3 = fragilidade (1–2 = pré-fragilidade); SARC-F ≥4 = suspeita de sarcopenia, confirmada por força de preensão baixa (<27 kg H / <16 kg M) + massa muscular reduzida.
- Base do tratamento partilhada: otimização proteica, vitamina D e exercício de resistência; identificação precoce é a chave para reverter fragilidade, sarcopenia e desnutrição.
Principle: Early identification of nutritional risk allows effective, low-cost dietary intervention, reducing complications, length of stay, readmissions and mortality. More than 30% of hospitalised patients are at nutritional risk.
Primary care
Screen every adult with an increased likelihood of nutritional deterioration:
- Age > 65 years
- Chronic disease that compromises nutritional status (COPD, cancer, renal, hepatic, gastrointestinal, inflammatory bowel disease) not followed by a hospital Nutrition service
- Advanced neurodegenerative disease
- Rehabilitation care, home care or institutionalised patients
- Clinical indicators: unintentional weight loss, anorexia, altered taste/smell, dysphagia, dental/chewing problems, fatigue/muscle weakness, pressure ulcers, recurrent infections, social isolation with compromised intake
Primary care frequency: at first contact; then yearly (no risk, not institutionalised) or every 3 months (institutionalised/home care).
Hospital care
- All patients admitted for > 24 h and cancer outpatients
- First assessment within the first 48 h after admission; repeat weekly if not followed by Nutrition
- Intensive care patients are considered always at risk (excluded from systematic screening)

IMPORTANT: Total score ≥ 3 = patient at nutritional risk/malnourished → start a nutritional care plan. Score < 3 → repeat screening weekly.
Indication for a nutritional care plan: severe malnutrition (=3); severe disease (=3); moderate malnutrition (=2) + mild disease (=1); mild malnutrition (=1) + moderate disease (=2).
NOTE: In wards with a low expected prevalence of risk, Table 1 alone may be applied. If anthropometric data or weight-loss history cannot be obtained reliably, consider the patient at risk until the next weekly assessment.

Malnutrition: a state resulting from insufficient intake/absorption of nutrients that alters body composition (loss of lean mass), with functional decline and worse prognosis. Screening (e.g. NRS 2002) always precedes diagnosis.
Three-step approach: (1) screening with a validated tool; (2) diagnosis requires ≥1 phenotypic criterion + ≥1 aetiologic criterion; (3) severity grading based on the phenotypic criteria.

CAUTION: Do not use hypoalbuminaemia as a marker of malnutrition (low sensitivity and specificity). Muscle strength (e.g. dynamometry) does not replace muscle-mass assessment in GLIM. Always assess all 5 criteria before ruling out the diagnosis.
Frailty: a syndrome of progressive decline across multiple physiological systems, with increased vulnerability to stressors. It is preventable and potentially reversible; screening is recommended for everyone aged ≥65.

Cachexia: a multifactorial syndrome with ongoing loss of muscle mass (with or without fat mass) that cannot be fully reversed by conventional nutritional support, associated with chronic inflammation. Screen for weight loss and anorexia in all cancer patients.

NOTE: Sarcopenia, malnutrition and cachexia share loss of muscle mass but differ in aetiology. Weight loss is not a criterion for sarcopenia (it is for malnutrition and cachexia). Malnutrition is a strong predictor of sarcopenia. They may coexist, particularly in older cancer patients.

- Alert system (Portuguese EHR, SClínico): yellow = screening missing; orange = risk identified without Nutrition assessment; grey = up to date
Monitoring indicators
- Proportion of patients screened within the first 48 h (target: ≥60%)
